Physical Therapist - Full Time - Home Health Care

Position Overview We are seeking a dedicated and experienced Physical Therapist to join our team at Pro Elite Physical Therapy Clinic in Fresno, California. This full-time position is centered around providing exceptional care in a home care setting, focusing on orthopedic rehabilitation. As a Physical Therapist, you will play a crucial role in helping patients regain their mobility and enhance their overall quality of life through personalized treatment strategies.

Pro Elite Physical Therapy Clinic is a leading provider of rehabilitative services committed to delivering the highest standard of care to our patients. Our Fresno location prides itself on tailored treatment plans that prioritize each individual's unique rehabilitation needs. The work setting is primarily onsite in the home care environment, where you will visit patients in their homes to provide therapy services. You will work Monday to Friday from 9 am to 5 pm, ensuring a work-life balance while engaging directly with patients in a comforting setting.

Manage a caseload efficiently, working with one patient per hour to ensure dedicated, individualized attention.

Utilize evidence-based practices to implement interventions that improve patients' physical capabilities while effectively reducing reliance on medications or surgical procedures.

Monitor and document treatment outcomes, adjusting care plans as necessary to optimize progress and maintain clear communication with patients and their families.

Collaborate with multidisciplinary healthcare professionals to ensure a holistic approach to patient care.

Maintain meticulous documentation of patient interactions and treatment services using EMR software, specifically WebPT.

Encourage and motivate patients throughout their recovery journey, fostering a supportive atmosphere that enhances their commitment to rehabilitation.

In this role, you will primarily work with patients recovering from orthopedic conditions, including post-surgical rehabilitation, sports injuries, and age-related mobility issues. A Bachelor's Degree in Physical Therapy is required. A Master's or Doctoral Degree in Physical Therapy is preferred.

Must possess a valid state license as a Physical Therapist with a clean professional record, free from any disciplinary actions.

Proficiency in manual therapy techniques and the development of individualized exercise protocols is essential.

Experience utilizing EMR software, particularly WebPT, to track patient progress and document care.

Prior experience in orthopedic outpatient settings, demonstrating a solid understanding of rehabilitation protocols and patient care strategies.
