Work Environment

This role is based in a home care setting, allowing you to work directly with patients in their environment, which enhances the quality and personalization of care. The work schedule includes weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs during weekends may be required. You will typically work an average of 40 hours per week, offering flexibility and opportunities to engage with the community.

Key Responsibilities

• Conduct thorough health assessments and screenings in patients' homes to identify medical issues and tailor care plans accordingly.
• Diagnose and manage a variety of acute and chronic medical conditions, implementing preventive care strategies and promoting overall health.
• Collaborate closely with local community organizations and stakeholders to design and deliver effective health education programs that address prevalent health concerns.
• Deliver culturally sensitive care while advocating for patients’ rights and ensuring equitable access to healthcare services.
• Coordinate patient referrals and follow-up care to maintain continuity in treatment and support.
• Maintain meticulous medical records in compliance with healthcare regulations, ensuring all documentation is up-to-date and accurate.
• Engage in community outreach activities, providing necessary health screenings and actively raising awareness about health issues affecting local populations.

Qualifications
