Work Environment

This position will be primarily onsite within the comfort of patients' homes, giving you the unique opportunity to provide therapy in a familiar environment. The clinic's operational hours are Monday to Friday, from 9:00 AM to 5:00 PM, ensuring a balanced work-life schedule.

Key Responsibilities

• Conduct comprehensive evaluations to assess mobility limitations, functional capabilities, and pain levels in each patient.
• Design and implement individualized treatment plans that address each patient's specific condition, goals, and preferences.
• Guide patients through prescribed rehabilitation exercises aimed at improving strength, flexibility, and overall function.
• Utilize advanced manual therapy techniques to alleviate pain and enhance physical function, modifying approaches as required to optimize outcomes.
• Regularly re-evaluate patient progress, adjusting treatment protocols as necessary to ensure sustained improvement and satisfaction.
• Maintain accurate and up-to-date documentation of patient care using EMR platforms, ensuring compliance and facilitating information sharing among the care team.

Patient Population / Client Focus

You will work closely with a diverse patient population, primarily focusing on individuals recovering from orthopedic injuries, surgeries, and chronic pain conditions. The supportive nature of home health care allows you to witness firsthand the impact of your work as you assist patients in navigating their rehabilitation journey, ultimately improving their quality of life.

Qualifications

• A Master's or Doctorate degree in Physical Therapy from an accredited institution.
• An active New Jersey Physical Therapist license is mandatory to practice.
• A minimum of 2 years of hands-on clinical experience in providing physical therapy, preferably in outpatient or home health settings.
• Demonstrated competency in manual therapy techniques and patient-centered care methodologies.
• Familiarity with electronic medical record (EMR) systems, particularly WebPT, to ensure efficient documentation and coordination of care.

Preferred Experience

• Experience working with patients in a home health care setting, understanding the unique challenges and dynamics of patient care in this environment.
• Specialized training or certifications in specific areas such as orthopedics, sports rehabilitation, or geriatric physical therapy.
• Proficient in collaborating with interdisciplinary teams, including physicians, occupational therapists, and nursing staff, to provide comprehensive patient care.
