Positive Change Therapy, LLC is seeking a dedicated and licensed Physical Therapist (PT) to join our team full-time at our Deltona, Florida rehabilitation facility. Key Responsibilities

• Conduct thorough evaluations of patients to establish comprehensive treatment plans that incorporate personalized goals and objectives while adhering to established standards of care.
• Deliver high-quality physical therapy sessions, employing advanced techniques including trigger point therapy to treat various conditions.
• Document all assessments, treatments, and billing information accurately within our electronic medical record system to ensure compliance and facilitate seamless patient care.
• Screen patients for potential risks such as lymphedema and falls, adjusting treatment plans as necessary to ensure optimal safety and effectiveness.
• Collaborate with an interdisciplinary team to address the multifaceted needs of patients, balancing individual care with broader health outcomes.
• Engage in departmental education efforts by participating in presentations, publications, and mentoring of students and interns to enhance clinical practice and career development.
• Maintain communication with patients' families, providing them with education regarding treatment goals and progress to foster strong support systems.

Patient Population / Client Focus

Our client base includes individuals experiencing functional limitations as a result of physical injuries, cognitive impairments, psychosocial dysfunctions, and various health challenges. You will work with both inpatient and outpatient populations across a wide range of ages and therapeutic needs, including pediatrics. Your role will be central to facilitating their recovery, enhancing mobility, alleviating pain, and preventing or minimizing disabilities.

Qualifications

• Current and valid Florida PT License is mandatory.
• A Bachelor's degree in Physical Therapy is required; a Master's or Doctorate degree in the field is preferred.
• A minimum of one year of clinical experience, preferably in an acute care hospital or rehabilitation setting.
• Strong fundamental knowledge of human anatomy and physiology.
• Competency in working with pediatric clients and an understanding of hospital-based practices.
• Ability to effectively provide direct patient care in both clinic and hospital environments.
• Familiarity with therapeutic modalities and procedures, particularly in trigger point therapy.

Preferred Experience

• Two or more years of clinical practice in a rehabilitation facility, with experience managing a high caseload of diverse patients.
• Experience with multi-disciplinary collaborative care and having taken part in departmental education efforts.
• Demonstrated capacity to handle administrative duties along with clinical responsibilities efficiently, ensuring minimal paperwork is necessary in daily practice.

• Competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications.
