Work Environment

This position is primarily onsite, where therapists work in the home environments of our patients. As part of a dynamic team, you will experience a flexible work setting, allowing you to select hours that align with your schedule, specifically Monday through Friday. Your role will involve traveling to various patient locations, providing therapy in familiar settings which can enhance patient comfort and engagement.

Key Responsibilities

• Evaluate and assess patients in their homes, developing individualized treatment plans aimed at achieving optimal recovery outcomes.
• Utilize a variety of therapeutic techniques, including exercises, manual therapy, and advanced rehabilitation equipment, to enhance mobility and alleviate pain.
• Closely monitor and document patient progress, ensuring that treatment goals are met in a timely manner and modifying plans as necessary based on patient response.
• Educate patients and their families about treatment processes, expected outcomes, and how to effectively use assistive devices when needed.
• Maintain comprehensive records of treatment sessions, including progress notes and treatment recommendations, ensuring compliance with state and federal documentation standards.
• Engage in ongoing professional development to enhance skills and stay updated with the latest physical therapy practices.

Patient Population / Client Focus

Your focus as a Home Care Physical Therapist will be to work with a varied patient demographic, including individuals recovering from surgery, injuries, or chronic illnesses. You will approach each case holistically, understanding that each patient's journey is unique and requires a customized treatment strategy. This reality necessitates a strong ability to connect with patients and their families, fostering trust and ensuring compliance with rehabilitation protocols.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid Physical Therapy license in the state of New Jersey is essential.
• Effective verbal and written communication skills are vital for patient education and documentation.
• Basic proficiency in computer processing skills for documentation and communication purposes.
• Must demonstrate flexibility and adaptability to meet the diverse needs of patients.

Preferred Experience

Compensation and Benefits
