Key Responsibilities\n

• Conduct thorough and ongoing assessments to address the specific needs of individual patients, utilizing evidence-based practices.

• Maintain consistent communication with supervisors, care team members, family, and other key individuals regarding patient progress and treatment adjustments.

• Develop and provide a tentative schedule to supervisors that outlines appointments, home visits, and observations for effective case management.
\n
• Coordinate and execute home visits, ensuring compliance with all necessary observations, chart reviews, and staff interviews to enhance our understanding of client situations.

• Oversee the effective implementation of prescribed program plans, ensuring that each patient receives the care outlined and necessary adjustments are made as needed.
\n
• Engage actively in Interdisciplinary Team Meetings, contributing insights and feedback to enhance collective patient care.
\n
• Complete detailed monthly and quarterly progress notes, documenting patient advancements and areas for continued focus.
\n
• Organize and deliver training to certify staff and team members on the implementation of action plans and associated documentation practices.
\n
• Provide ongoing support through follow-up interviews, additional home visits, and observations to address evolving patient needs.
\n
• Maintain meticulous records to ensure transparency and availability of information to relevant stakeholders.

• Act as a liaison within the mental health community, fostering relationships that facilitate resource sharing and support for developmental goals.

• Assist in the creation of procedural materials aimed at staff training, enhancing program efficacy, and improving patient outcomes.

Patient Population / Client Focus\n

This role primarily involves working with a diverse client base, including both adults and youth. The LPC will cater to individuals facing various mental health challenges, including but not limited to anxiety disorders, depression, substance misuse, trauma, and family dynamics. A nuanced understanding of the unique needs of these populations is crucial for successful engagement and therapeutic outcomes.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Licensed Professional Counselor (LPC) within Arizona.
