MediStar Mental and Counseling Services is seeking a dedicated and skilled Licensed Mental Health Therapist to join our team in Mount Vernon, New York. In this full-time remote role, you will utilize telehealth technologies to extend compassionate, evidence-based mental health care to a diverse array of patients, enabling them to manage their mental health in conjunction with any co-occurring medical conditions.

Key Responsibilities

• Conduct in-depth mental health assessments with a keen understanding of the interplay between mental and physical health conditions.
• Develop and implement individualized treatment plans that integrate both psychiatric and medical perspectives, ensuring comprehensive care.
• Facilitate both individual and group therapy sessions, utilizing evidence-based techniques to foster patient growth and recovery.
• Work collaboratively with medical providers to coordinate care and optimize treatment pathways, sharing insights and observations from therapy sessions.
• Provide timely crisis intervention and support during emergencies, demonstrating a strong ability to respond to volatile situations compassionately.
• Empower patients through education on self-management strategies and the relationship between mental health and physical wellbeing.
• Ensure accurate and confidential documentation of patient records in accordance with legal and ethical standards, utilizing electronic health record systems effectively.
• Engage actively in team meetings and case conferences, contributing to interdisciplinary care planning and continuous team development.
• Stay informed about advancements in integrated care and best practices, contributing innovative ideas to improve service delivery.

Patient Population / Client Focus

You will work primarily with individuals experiencing complex challenges due to co-occurring medical and mental health conditions. This includes patients grappling with chronic diseases, substance use disorders, and those needing assistance managing the psychological aspects of their physical health journeys. Your role is crucial in helping them navigate these challenges, ensuring they receive a holistic treatment approach.

Qualifications

• A Master's or doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or equivalent.
• Proficiency in addressing the mental health needs of patients with concurrent medical conditions is essential.
• Effective interpersonal and communication skills, with the ability to build trust and rapport with patients quickly.
• Strong adherence to ethical guidelines and professional boundaries, ensuring patient confidentiality.
• Familiarity with electronic health record (EHR) systems and clinical documentation practices.

Preferred Experience

• Experience in a telehealth or outpatient mental health setting.
• Background in integrated care models and interdisciplinary teamwork.
• Exposure to crisis intervention and management.

Compensation and Benefits
