Work Environment

This position is primarily onsite in the homes of our patients located in Cypress and surrounding areas. As a Home Care Physical Therapist, you will enjoy the autonomy of conducting therapy sessions in a comfortable, familiar environment for the patient. All necessary equipment is provided, and you will be equipped with state-of-the-art tools to enhance treatment effectiveness. Key Responsibilities

• Perform thorough evaluations of patients' physical abilities and limitations to create tailored treatment plans that cater to their specific needs and medical history.
• Implement rehabilitation programs utilizing various treatment techniques, including therapeutic exercises, manual therapy, and modalities to alleviate pain and promote mobility.
• Continuously monitor patient progress and adjust treatment plans as required, ensuring that therapeutic goals evolve according to patient needs.
• Educate patients and their families on exercise regimens, self-care strategies, and injury prevention to foster independence and enhance their quality of life.
• Document all patient evaluations, treatment plans, progress notes, and outcomes meticulously, ensuring compliance with health regulations and documentation standards.
• Collaborate effectively with caregivers, family members, and medical professionals to ensure a coordinated approach to patient care.
• Stay updated on the latest advancements in physical therapy by participating in workshops, professional networks, and continuing education opportunities.

Patient Population / Client Focus

You will be working with a diverse patient population that includes children and adults recovering from orthopedic injuries, surgeries, and chronic illnesses. Qualifications

• A Bachelor's degree in Physical Therapy (Doctorate in Physical Therapy (DPT) preferred).

• A minimum of 2 years of clinical experience in physical therapy, preferably with a focus on orthopedic rehabilitation.
• Excellent communication skills, both written and verbal, for effective patient interaction and documentation.

Preferred Experience
