Work Environment\n

This position is remote, allowing for flexibility in scheduling while providing essential on-site services to seniors in their homes. The work hours typically span Monday to Friday from 8:00 AM to 4:30 PM, with some evening or weekend hours as needed to accommodate client schedules. The LCSW will be required to engage in occasional on-call duties to ensure timely support for urgent situations. Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate the emotional, mental, and social factors affecting seniors and their families.

• Regularly monitoring client well-being through home visits, reassessing their living conditions, and identifying any safety concerns.

• Facilitating communication among family members involved in the care of the senior client to minimize conflicts and ensure a unified care approach.

• Maintaining accurate and confidential documentation of client interactions, assessments, and care plans to adhere to regulatory standards.

Patient Population / Client Focus\n

The primary focus of this role is on seniors with advanced illnesses who require support during their transition to hospice care. This includes physical, emotional, and social aspects of end-of-life care, and involves working closely with families to provide emotional support and guidance through a difficult time.

Qualifications\n

To be successful in this role, candidates must possess:

• A Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Florida.

• A minimum of two years of experience in a social work role, ideally within a hospice, geriatric, or community health setting.

Compensation and Benefits\n
