Key Responsibilities\n

• Conduct one-on-one sessions with clients to explore their goals, needs, and preferences, laying the groundwork for personalized rehabilitation plans.

• Develop detailed, individualized rehabilitation plans that reflect each client's education level, unique capabilities, and aspirations, refining these documents as the clients’ needs evolve.
\n
• Collaborate closely with a multidisciplinary team—comprising physicians, occupational therapists, and psychologists—to ensure that each rehabilitation program is tailored to address specific client challenges effectively.
\n
• Maintain accurate and up-to-date case files that include personal information, treatment history, eligibility, and client interactions, ensuring compliance with all confidentiality and documentation requirements.

• Monitor client progress meticulously, adapting treatment goals and strategies as necessary to foster success and ensure effective interventions.
\n
• Engage routinely with clients participating in job training and on-site programs, addressing any arising concerns quickly and effectively.
\n
• Work collaboratively with clients’ family members or other significant individuals to identify and understand the best approaches to meeting their needs.
\n
• Conduct timely analysis and documentation of client needs and progress, providing insightful reports and recommendations for additional resources as required.

Patient Population / Client Focus\n

This role will primarily focus on clients coping with a range of mental health and mood disorders, including but not limited to depression, anxiety, and adjustment disorders. You will support both individuals seeking to improve their mental health and those looking to reintegrate into their communities or the workforce through tailored rehabilitation services.

Qualifications\n

• A completed Master’s Degree in Counseling, Social Work, Psychology, or Marriage and Family Therapy.

• Active Licensed Professional Counselor (LPC) licensure.
\n
• Proven experience working with mental health disorders in either residential or outpatient environments.
\n

• Demonstrated proficiency in clinical methodologies, including motivational interviewing, cognitive-behavioral therapy, and dialectical behavioral therapy.

Preferred Experience\n

Experience utilizing electronic health record systems for documentation and tracking treatment progress is a plus. Familiarity with telehealth platforms and remote counseling best practices will enhance your transition into this role. Candidates with certifications in specialized therapeutic techniques will be highly regarded.
