Key Responsibilities

• Develop and implement customized treatment plans for patients aimed at preventing disability, relieving pain, restoring mobility, and enhancing functional abilities.
• Oversee and guide Physical Therapy Assistants in the execution of treatment plans, ensuring high standards of care are met.
• Utilize a variety of assessment tools to evaluate the client's functional capabilities and adjust treatment strategies as necessary.
• Conduct thorough evaluations of patients' home environments to identify barriers to independence and recommend modifications to enhance rehabilitation.
• Administer therapeutic modalities, manual therapy, and exercise programs, adapting approaches to meet each patient's unique needs.
• Prepare and maintain accurate documentation, including initial evaluations, progress notes, and discharge summaries, in compliance with regulatory requirements.
• Collaborate with an interdisciplinary team during case conferences to refine treatment plans and improve overall patient outcomes.
• Educate patients and their families on physical therapy techniques and the appropriate use of equipment in their recovery journey.
• Regularly assess the effectiveness of the treatment plans and make modifications as necessary, communicating changes with healthcare providers and case managers.
• Participate in ongoing professional development opportunities and team training sessions to stay updated with the latest advancements in physical therapy practices.

Patient Population / Client Focus

You will work with a diverse patient population ranging from post-surgical clients to those requiring rehabilitation for chronic conditions. Our clients often seek treatment to recover from sports injuries, orthopedic surgeries, stroke rehabilitation, and other musculoskeletal issues. Qualifications

• A Bachelor's Degree in Physical Therapy or higher from an accredited institution.
• Current and active Physical Therapist license in the State of Florida.

• Proficiency in electronic medical records (EMR) software for documentation and patient management.
• Excellent communication and interpersonal skills to effectively interact with patients, families, and other healthcare professionals.

Preferred Experience

• Experience in a rehabilitation setting with a focus on outpatient physical therapy preferred.

• Previous experience mentoring or supervising Physical Therapy Assistants is a plus.

Compensation and Benefits
