Key Responsibilities

In this role, your responsibilities will include:

• Directing comprehensive physical therapy treatment plans that aim to reduce pain, address mobility issues, and support client rehabilitation goals.
• Overseeing and mentoring Physical Therapy Assistants to ensure effective implementation of the care plans and therapeutic exercises.
• Conducting thorough evaluations using diagnostic assessments to determine the current functional capabilities of clients.
• Assessing the home environment of clients to provide tailored recommendations that promote safety and independence.
• Administering specialized treatments such as manual therapy techniques, traction, and the application of physical agents like heat and cold.
• Documenting all patient interactions, creating detailed clinical records—including OASIS evaluations, progress notes, and discharge summaries—and ensuring proper communication with the interdisciplinary care team.
• Participating actively in interdisciplinary team meetings, showcasing a commitment to collaborative approaches in developing and adjusting client care plans.
• Providing education and training on home exercise programs and the utilization of physical therapy devices to patients and their families.
• Completing other assignments as directed to achieve organizational goals and objectives.

Patient Population / Client Focus

The patients you will serve are primarily individuals requiring home-based rehabilitation following surgery, injury, or chronic conditions impacting their mobility and daily living activities. This includes elderly patients, post-operative recovery cases, and others in need of personalized therapy to regain independence in their homes. Your ability to connect with patients and build relationships based on trust and respect will be essential to their therapeutic success.

Qualifications

To be successful in this position, we require:

• A Doctorate in Physical Therapy (DPT) and a valid license to practice in Virginia.
• At least two years of experience as a physical therapist, with a preference for candidates with experience in home care or outpatient settings.
• Current CPR certification and an immaculate driving record, including valid driver's license and automobile insurance.
• A commitment to clinical excellence, outstanding communication skills, and the ability to work collaboratively within a team and with families.
• A strong ethical compass, demonstrating integrity in all aspects of patient care.

Preferred Experience

We prefer candidates who have:

• Previous experience working in a home healthcare environment or similar settings.
• Demonstrable skills in innovative thinking and problem-solving, allowing for enhanced patient care practices.
• Familiarity with documentation requirements and federal/state regulations concerning home health therapy services.

Compensation and Benefits

The annual salary for this position ranges from $100,000 to $120,000, commensurate with experience and qualifications. In addition to competitive compensation, we offer:
