Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, you will be responsible for:

• Developing and implementing personalized treatment plans that align with each client's unique needs and functional goals, monitored through regular assessments.
• Overseeing and mentoring Physical Therapy Assistants to ensure the appropriate execution of therapeutic interventions as per prescribed care plans.
• Conducting thorough evaluations of patient capabilities using a variety of diagnostic and prognostic tests to establish functional baselines.
• Assessing home environments to identify barriers to mobility and independence while providing recommendations for modifications or assistive devices.
• Administering a range of therapeutic modalities, including manual therapy, therapeutic exercises, and physical agents such as traction and massage.
• Collaboration during interdisciplinary team case conferences to ensure comprehensive care coordination, progress tracking, and adjustment of treatment plans as needed.
• Documenting clinical evaluations, progress notes, and treatment outcomes in compliance with regulatory standards, ensuring timely submission of required documentation.
• Providing education and guidance to clients and their families about rehabilitation programs, physical therapy devices, and preventative strategies for ongoing health.
