Work Environment

This role is based onsite, primarily in the homes of clients across Richmond, Virginia. As a Physical Therapist, you will have the flexibility to manage a varied caseload in a community setting. Working relationships with interdisciplinary teams, including Case Managers and Physical Therapy Assistants, will be essential. Key Responsibilities

In this position, you will take on the following responsibilities:

• Develop and execute individualized treatment plans aimed at preventing disability, relieving pain, and enhancing muscle function in alignment with physician directives.
• Conduct evaluations of clients' physical abilities using established diagnostic tests, ensuring accurate assessments of their functional capacities.
• Evaluate the home setting for each client, making actionable recommendations to enhance their independence and rehabilitation potential.
• Provide hands-on physical therapy services, including manual therapeutic exercises, traction, and massages, while also instructing clients in non-manual activities for functional improvements.
• Collaborate with Physical Therapy Assistants, ensuring they are equipped to support the execution of care plans and are supervised as required by state law.
• Maintain meticulous clinical records, including evaluations, progress notes, and care plans, ensuring compliance with OASIS guidelines.
• Engage in case conferences to review treatment strategies collaboratively and keep lines of communication open among interdisciplinary team members, clients, and their families.
• Complete functional assessments onsite and adapt care plans based on evolving patient needs and available community resources.
• Stay accessible to provide guidance to Physical Therapy Assistants in real-time during client interactions.
• Adhere to all documentation and reporting policies, ensuring timely submission that meets organizational needs and standards.

You will primarily work with a diverse client base, including elderly patients, individuals recovering from surgeries, and those managing chronic conditions. Qualifications

Qualified candidates will possess:

• A Doctorate Degree in Physical Therapy (DPT) from an accredited institution.

Compensation and Benefits
