Key Responsibilities\n

• Collaborate with healthcare team members to develop and implement comprehensive treatment plans, including discharge planning that addresses both psychosocial and clinical needs.

• Conduct thorough psychosocial assessments for patients, utilizing standardized tools and methodologies to identify emotional, behavioral, and environmental factors influencing their health.

• Maintain accurate and timely documentation, including clinical notes and progress reports, in accordance with department policies and electronic medical record (EMR) systems.
\n
• Provide individual and group counseling to patients, employing evidence-based practices to address emotional and behavioral challenges, while facilitating crisis intervention when necessary.

• Engage with family members and support networks to enhance treatment outcomes and ensure collaborative discharge planning, offering resources and referrals as needed.

Patient Population / Client Focus\n

The primary focus of this role is to support individuals facing a myriad of challenges including mental health concerns, substance use issues, and significant life transitions. You will work with a diverse patient population, which includes young adults, families, and professionals from various backgrounds. Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• A minimum of 2 years of professional experience in a clinical setting, preferably in a healthcare or community health context.

• Competency in using electronic medical records and proven skills in word processing, email communication, and data management.
