Work Environment

This position is based onsite in Lynbrook, New York, where our healthcare providers work directly with patients through concierge medicine and house calls. Rather than a traditional clinical setting, you will engage with patients in their homes or communities, offering personalized care tailored to their unique needs. Key Responsibilities

• Manage both the physical and psychological aspects of patients' cardiovascular issues, ensuring holistic care tailored to individual needs.
• Conduct thorough evaluations, identifying normal and abnormal findings in patients’ cardiovascular health.
• Develop, implement, and modify personalized treatment plans based on ongoing assessments and patient feedback.
• Monitor and evaluate patients' physical and emotional responses to medications and treatments, adjusting care strategies as necessary.
• Prescribe immunizations in accordance with established schedules to enhance the health and wellness of patients.
• Collaborate effectively with physicians and other healthcare professionals to coordinate patient care and achieve optimal health outcomes.
• Maintain comprehensive and accurate patient records, ensuring compliance with documentation standards at each patient encounter.
• Lead patient education efforts, providing essential information about cardiovascular health to empower patients in their care journeys.

Patient Population / Client Focus

This role specifically focuses on patients experiencing cardiovascular conditions, ranging from hypertension to complex heart diseases. By working closely with cardiologists, you will be responsible for the assessment and management of patients across diverse demographics, ensuring that care is culturally sensitive and accessible. Qualifications

• Master's degree in Nursing, specifically in Nurse Practitioner (NP) from an accredited program.
• Current Registered Nurse (RN) License issued by the State of New York.
• Basic Life Support (BLS) Certification is required.

• 2 to 3 years of experience in a cardiology-focused role is preferred, providing familiarity with cardiovascular conditions and treatments.
• Experience working in concierge medicine or house call settings is a plus.
• Demonstrated strong interpersonal and communication skills, with the ability to connect effectively with diverse patient populations.

Compensation and Benefits

The offered salary for this position ranges from USD 150,000 to 160,000 annually, accompanied by competitive benefits.
