Key Responsibilities

• Implement targeted physical therapy interventions to alleviate pain and improve functional muscle abilities in accordance with individualized care plans prescribed by physicians.
• Evaluate clients' physical capabilities using diagnostic tests and clinical assessments to inform treatment strategies effectively.
• Conduct comprehensive home assessments to recommend necessary modifications that enhance safety and independence for clients undergoing rehabilitation.
• Provide hands-on therapeutic treatment, including manual exercises, functional activities, and modalities such as traction and massage to aid recovery.
• Coordinate with physical therapy assistants, ensuring they adhere to the prescribed care protocol and deliver services effectively while under your guidance.
• Maintain thorough clinical documentation, including progress notes, initial evaluations (OASIS), and discharge summaries, to ensure accurate patient records and continuity of care.
• Engage in interdisciplinary team meetings to evaluate client progress collaboratively, integrating insights from various healthcare professionals.
• Educate clients and their families about treatment plans, rehabilitation devices, and overall wellness strategies for at-home support.
• Monitor changes in client conditions, proactively suggesting necessary adjustments to treatment plans in consultation with case managers and physicians.
• Exhibit professionalism and integrity while representing the organization and advocating for clients' needs within the home healthcare framework.

Patient Population / Client Focus

The clients you will serve range from seniors seeking assistance in maintaining their independence to individuals recovering from orthopedic surgeries or injuries. Qualifications

• A Doctor of Physical Therapy (DPT) degree is required.
• Active and valid PT licensure in the state of California is mandatory.
• At least one year of experience as a physical therapist, preferably in home care or acute care settings.
• Current CPR certification.
• Possession of a valid driver's license and reliable transportation necessary to visit clients.
• Strong commitment to maintaining the highest standards of clinical excellence and patient engagement.
• Ability to work collaboratively within a multidisciplinary team and communicate effectively with diverse patient populations.
• Proficient knowledge of federal and state regulations governing home healthcare and physical therapy practices.
• Excellent interpersonal and written communication skills.

Preferred Experience

While a minimum of one year of experience is required, candidates with additional hands-on experience in home healthcare or specialized training in orthopedic physical therapy will be viewed favorably. Compensation and Benefits

We offer a competitive salary range of USD 90,000 - 110,000 per year, commensurate with experience and qualifications.
