Work Environment

This position is based onsite, specifically within the home care setting. Key Responsibilities

As an Orthopedic Physical Therapist at Reliant, your responsibilities will include:

• Conducting comprehensive evaluations of patients to assess their physical condition, functional abilities, and rehabilitation needs.
• Designing and implementing individualized treatment plans that focus on achieving specific rehabilitation goals and optimizing patient recovery.
• Administering hands-on physical therapy techniques, while also teaching patients adaptive exercises and techniques to promote independence in their recovery.
• Collaborating with physicians and the patient care team to refine treatment approaches, ensuring all aspects of care align with the overall goals for the patient.
• Monitoring patient progress regularly and adjusting treatment plans as needed based on individual patient responses and progress.
• Providing patients and their families with education on prosthetic devices, therapeutic exercises, and coping strategies to enhance home management.
• Managing a caseload effectively, prioritizing patient care while meeting daily staffing requirements and maintaining clear communication with other team members.

Patient Population / Client Focus

You will be primarily working with individuals recovering from orthopedic surgeries or injuries, encompassing a diverse patient population. Your expertise will be vital for patients of all ages, requiring specialized attention to their unique needs and recovery challenges. Success in this role involves building strong therapeutic relationships that enhance patient motivation and engagement in their treatment plans.

Qualifications

• A current and active Physical Therapy License in the state of New York.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Certification in Basic Cardiac Life Support (BCLS), or willingness to obtain within the first 30 days of employment.
• Strong communication and interpersonal skills that facilitate effective patient interactions and education.

Preferred Experience

Compensation and Benefits
