Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to gauge the developmental status of children and provide in-depth reports to the interdisciplinary team.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that family goals and objectives are prioritized.
\n
• Effectively monitor and document outcomes, collaborating with other healthcare professionals to track each child's progress and adapt strategies as needed.
\n
• Provide ongoing support to families, actively engaging them in interventions that benefit their child's growth and development.
\n
• Adhere to all relevant federal and state regulations, maintaining compliance to guarantee high standards of care.
\n
• Establish strong rapport with children, families, and team members to create a nurturing and productive learning environment.
\n
• Utilize evidence-based practices to develop tailored strategies for treatment, leveraging skills in natural environment teaching.
\n
• Educate families on enhancing their child's physical, cognitive, social/emotional, and communication skills.
\n
• Utilize the coaching model to ensure that interventions are carried out in natural learning settings.
\n
• Maintain a safe and healthy environment for all staff and those we serve, upholding the highest standards of care.

Patient Population / Client Focus\n

In this role, you will concentrate primarily on infants and toddlers who are experiencing developmental delays or disabilities. Your efforts will not only target their individual growth but also support their families, equipping them with the necessary tools to foster an enriching home environment. Each child's care plan will be customized, reflecting their unique needs and family priorities, ensuring that all interventions are aligned with their development journey during these crucial early years.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) through the Department of Public Health.

• A minimum of one year of professional experience in early intervention services with children.

• A valid driver's license is required for occasional travel to assist families as needed.
\n
• Strong organizational and time management skills, enabling effective case management and documentation.

Compensation and Benefits\n
