Key Responsibilities

• Develop and implement individualized treatment plans based on comprehensive evaluations, ensuring alignment with physician protocols and best practices.
• Oversee and mentor Physical Therapy Assistants, ensuring accurate delivery of therapeutic services as per the established care plans.
• Conduct detailed assessments of patients to determine their functional status, using various diagnostic and prognostic tests related to muscle, nerve, and joint functions.
• Evaluate patients' home environments and make tailored recommendations to increase their independence and promote a safe living situation.
• Administer therapeutic exercises, traction, and massage treatments, providing guidance and support to patients during their therapy sessions.
• Take responsibility for documenting patient progress and changes in condition, ensuring timely updates are communicated to physicians and case management teams.
• Participate actively in interdisciplinary team meetings to discuss treatment strategies and evaluate the overall effectiveness of care.
• Provide education to patients and their families regarding the therapy process, equipment use, and ongoing maintenance of their rehabilitation plans.
• Maintain clinical records meticulously, including initial evaluations, care plans, progress notes, and discharge summaries, as required by federal and state regulations.
• Continuously assess ongoing needs within the facility, identifying areas for improvement and advocating for necessary resources.
• Be accessible to support Physical Therapy Assistants during treatment sessions, ensuring adherence to care objectives and safety protocols.

Patient Population / Client Focus

In this role, you will primarily work with patients recovering from surgeries, sports injuries, neurological conditions, and age-related mobility challenges. Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Minimum of one year of experience working in a physical therapy capacity, preferably in an acute care or home care setting.
• Active Physical Therapist licensure in New Jersey, maintained in good standing.
• Current CPR certification indicating proficiency in emergency response.
• Strong interpersonal and communication skills, enabling effective collaboration with interdisciplinary team members and patient engagement.

Compensation and Benefits

The salary for this full-time position ranges from $100,000 to $120,000 annually, based on experience and qualifications.
