Work Environment

The Physical Therapist position is situated within our vibrant rehabilitation facility in South River. Key Responsibilities

• Direct the physical therapy treatment for patients, utilizing proactive methods to prevent disabilities, alleviate pain, and improve muscle function, always adhering to individualized plans of care.
• Supervise and guide Physical Therapy Assistants to guarantee the effective implementation of treatment plans.
• Conduct thorough evaluations of patient capabilities using a variety of diagnostic tools to assess functional abilities related to muscle and joint performance.
• Examine and modify home environments to support rehabilitation goals, making recommendations that will optimize patient safety and independence.
• Deliver therapeutic exercises and treatments, including traction, massage, and application of physical agents, ensuring all activities align with prescribed medical directives.
• Provide instruction and support to clients and their families on the effective use of physical therapy devices and participate actively in educational sessions about their rehabilitation plans.
• Maintain comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring adherence to organizational policies and compliance with regulations.
• Engage in interdisciplinary team meetings to collaboratively plan and evaluate patient treatment strategies.
• Monitor any changes in patient condition, documenting observations meticulously and communicating with the referring physician and case manager as necessary.
• Complete functional assessments, adjusting treatment plans as required based on client progress and community resource utilization.
• Contribute to an environment of continuous improvement by taking on additional responsibilities and participating in company initiatives.

Patient Population / Client Focus

You will work primarily with a diverse patient demographic which includes those recovering from surgical interventions, managing chronic aches and pains, and rehabilitating after sports-related injuries. Qualifications

• Possession of a Bachelor's Degree in Physical Therapy or a relevant field from an accredited institution.
• A minimum of one year of experience as a licensed Physical Therapist in an acute care or home care setting.
• Current PT licensure in good standing for the state of New Jersey.
• Valid CPR certification to ensure patient safety during care.
• A strong commitment to clinical excellence, coupled with a genuine passion for patient care.
