Key Responsibilities

• Elicit an in-depth assessment of patients recovering from injuries, illnesses, or surgeries, conducting evaluations in a supportive home care environment.
• Develop tailored treatment plans that leverage a variety of rehabilitation techniques, including exercise regimens, manual therapy, and the use of specialized equipment to alleviate patient discomfort and enhance mobility.
• Continually document and monitor patient progress, adjusting treatment strategies as required to meet therapeutic goals efficiently.
• Train patients and their families on effective self-care practices and the proper use of assistive devices, leading workshops and one-on-one sessions for optimal understanding and compliance.
• Collaborate closely with interdisciplinary healthcare teams to ensure comprehensive care pathways and patient-centered outcomes.
• Engage in ongoing professional development through educational workshops, contributing to team knowledge sharing and best practice improvements.

Qualifications

Compensation and Benefits
