Key Responsibilities\n

• Conduct comprehensive evaluations and psychosocial assessments of seniors, focusing on their emotional and social needs to develop personalized care plans.

• Engage in multidisciplinary team meetings to collaborate on Individualized Family Service Plan (IFSP) development, implementation, and monitoring, ensuring the team's collective goals are met.

• Provide ongoing counseling and support to clients and their families, advocating for their needs and strengthening their emotional resilience during this challenging time.

• Guide families through the complexities of hospice care, offering education on resources, coping strategies, and support networks.

• Ensure compliance with all federal and state regulations regarding social work practice and documentation standards.

Patient Population / Client Focus\n

In this role, you will predominantly work with elderly clients facing terminal illnesses and their families. Qualifications\n

• A Master's degree in Social Work (MSW) or a related field, from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.

• Minimum of 2 years of clinical experience in a healthcare setting, demonstrating expertise in assessment, diagnosis, treatment planning, and crisis management.

• Exceptional verbal and written communication skills, with the ability to articulate complex concepts clearly to clients and their families.

• Strong organizational skills with a demonstrated ability to manage a caseload effectively and document case notes per standards.

• Understanding of age-related issues and the developmental needs of the populations served.
