Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for hospice patients through in-depth interviews, observations, and careful review of medical records to identify emotional, social, and environmental issues impacting patients’ care.

• Provide empathetic counseling and grief support for patients and their families to help them navigate adjustment issues related to illness, end-of-life care, and bereavement.

• Collaborate closely with a multidisciplinary rehabilitation team to devise individualized treatment plans that meet the psychosocial needs of each patient, ensuring a holistic approach to care.

Qualifications\n

Compensation and Benefits\n
