Key Responsibilities\n

• Utilize your education, training, and clinical expertise to conduct independent psychosocial evaluations and diagnoses for patients in hospice care.

• Collaborate with patients and their families to develop comprehensive, patient-centered treatment plans tailored to each individual’s unique circumstances and goals.

• Implement evidence-based behavioral interventions, including but not limited to solution-focused therapy, motivational interviewing, and problem-solving strategies, to enhance patient coping skills.

• Facilitate multi-disciplinary care coordination by partnering with primary care providers, psychologists, and other professionals to create a seamless and integrated approach to patient health.

• Monitor treatment effectiveness and patient progress through systematic tracking and regular follow-ups, whether in person or through virtual platforms.

• Support patients in managing their psychotropic medications as per the guidelines set by medical providers, focusing on adherence to prescribed regimens, recognizing side effects, and assessing overall treatment impact.

Patient Population / Client Focus\n

Your role will primarily involve working with patients receiving hospice care, who often experience complex emotional and psychological challenges associated with terminal illness. This could include addressing anxiety, grief, depression, and family dynamics. Your expertise will empower these patients and their loved ones to navigate their unique journeys with dignity and support.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in New Jersey.

• A minimum of 1 year of experience as an independently licensed therapist, with a focus on mental health care and hospice services preferred.

Experience in a hospice or palliative care setting is highly desirable. Compensation and Benefits\n

We offer a competitive salary range of $80,000 to $100,000 annually, complemented by an annual incentive bonus.
