Work Environment

This role is designed to be performed remotely, allowing for flexibility in your work schedule while maintaining a focus on engaging effectively with clients and collaborating with team members. Our typical operational hours span Monday to Friday, 9:00 AM to 5:00 PM, fostering a balanced work-life dynamic. Key Responsibilities

• Act as a critical liaison between families, clients, and outpatient healthcare providers, ensuring continuity of care and effective communication.
• Navigate interactions with insurance companies to facilitate pre-certification and oversee utilization management processes.
• Participate actively in discharge and aftercare planning, helping clients transition smoothly from acute care to their home environments.
• Provide therapeutic meal support, guiding clients in developing a healthy relationship with food while demonstrating positive eating behaviors.
• Deliver empathetic and affirming care that recognizes and respects the unique intersecting identities of each client, ensuring personalized treatment approaches.

Patient Population / Client Focus

Your expertise will be vital in working with a diverse clientele, including older adults who often face unique challenges related to mental health and social well-being. Qualifications

• A Master’s degree in Social Work from an accredited institution is required for this position.
• Candidates must hold a valid LCSW license in Tennessee.

Compensation and Benefits
