Key Responsibilities\n

  Collaborate with physicians and other mental health professionals to communicate patient progress, discuss needs and concerns, and address potential challenges related to treatment.

  Consult and coordinate with families and support systems to develop comprehensive treatment and discharge plans tailored to each client’s unique circumstances.

  Maintain accurate and timely documentation of all clinical activities, including progress notes, treatment plans, and follow-up care, in accordance with departmental policies and electronic medical records.

  Develop treatment strategies in collaboration with the healthcare team, based on thorough psychosocial assessments and evidence-based practices.

  Assist clients with discharge planning and care coordination to ensure a smooth transition back into their daily lives, leveraging community resources and regional services.

Patient Population / Client Focus\n

The ideal candidate will work with a diverse population, including individuals dealing with anxiety, depression, trauma, and substance use disorders. Our clients often face complex life challenges, and our counseling approach emphasizes understanding their unique backgrounds and experiences. Qualifications\n

  Master’s degree in Social Work, Counseling, Marriage & Family Therapy, Psychology, or a related field.

  Active state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or a corresponding license in line with state regulations.

  A minimum of three years of relevant experience in an outpatient mental health setting, showcasing strong clinical skills.

  Proficient in using various computer systems for record keeping, including word processing, email communication, spreadsheets, and electronic medical records management.

Compensation and Benefits\n
