Key Responsibilities\n

• Hold a caseload of youth clients; establish and maintain positive, therapeutic relationships while adhering to professional ethics and boundaries.

• Conduct regular sessions to monitor progress, review treatment goals, and facilitate discharge planning when appropriate.

• Create, implement, and track person-centered treatment plans that align with each client's unique needs and objectives.

• Document client interactions and progress notes accurately and in a timely manner to ensure continuity of care.

• Complete intake assessments to gather relevant information that informs treatment strategies.

• Engage in individual supervision and participate in team meetings to enhance clinical skills and collaborative practices.

• Work collaboratively with other healthcare providers to coordinate care and ensure comprehensive support for clients.

Patient Population / Client Focus\n

Qualifications\n
