Key Responsibilities\n

• Develop and maintain a tentative schedule of appointments, home visits, and observations to ensure proactive monitoring of client advancement.

• Coordinate in-home observations, conduct chart reviews, and lead staff interviews to gather comprehensive data for case evaluation and treatment planning.

• Implement and monitor individualized program plans, ensuring that therapeutic strategies are appropriately executed and aligned with patient goals.

• Participate actively in interdisciplinary team meetings to share insights, collaborate on treatment strategies, and stay updated on organizational developments.

• Prepare and complete detailed monthly and quarterly progress notes to document client growth and identify areas for adjustment in treatment approaches.

• Organize and facilitate training sessions to certify staff competency in program implementation and documentation processes, enhancing overall service delivery.
