Key Responsibilities Conduct comprehensive mental health assessments for patients, which include psychosocial evaluations, and risk assessments to identify their unique needs. Collaborate effectively with patients to create individualized treatment plans that are tailored to their specific goals and cultural contexts, promoting overall well-being. Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT), in various settings including individual, couple, family, and group therapy sessions. Provide crisis intervention services for patients in distress, working closely with a group of healthcare professionals to ensure comprehensive continuity of care. Maintain meticulous clinical documentation, including progress notes and discharge summaries, ensuring compliance with regulatory guidelines and organizational policies. Offer psychoeducation to patients and their families, empowering them with strategies and tools to manage mental health conditions and cope with end-of-life challenges. Engage in regular case reviews with a multidisciplinary team, including medical doctors, psychologists, and nurses, to coordinate and optimize patient care strategies. Conduct outcome assessments to evaluate the effectiveness of treatment plans and adjust strategies as needed to enhance patient outcomes. Continuously update your knowledge of best practices and emerging trends in mental health care, integrating new insights into your therapeutic approaches. Uphold ethical standards throughout all patient interactions, maintaining strict confidentiality at all times. Patient Population / Client Focus This role specifically caters to patients in hospice care, focusing on those nearing the end of life. You will directly support both patients and their families, providing emotional and mental health assistance that helps them navigate complex feelings associated with terminal illnesses. Qualifications A Master's degree in Social Work (MSW) is required. Possession of an active State license as a Licensed Clinical Social Worker (LCSW). Proven ability to work independently while also being an effective collaborator within a multidisciplinary team environment. Strong interpersonal and communication skills, with an emphasis on building rapport with individuals from diverse cultural backgrounds. Preferred Experience Experience in hospice or palliative care settings, particularly in mental health support. Familiarity with electronic health record (EHR) systems for documentation and care coordination. Knowledge of community resources for patients and families in need of additional support. Compensation and Benefits The annual salary for this position ranges from USD 70,000 to USD 84,000, with actual compensation depending on geographic location, work experience, educational background, and skills.
