Key Responsibilities

As a Physical Therapist, you will be responsible for:

• Developing and executing individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring muscle function based on physician referrals.
• Conducting comprehensive evaluations using diagnostic tests to assess a client's physical capabilities, enabling precise treatment planning.
• Assessing client home environments for rehabilitation needs and making actionable recommendations to enhance independence.
• Administering therapeutic exercises, traction, and other physical agents while motivating clients to engage in their rehab programs.
• Educating clients and their families about therapeutic devices and the overall physical therapy plan.
• Preparing and maintaining thorough clinical documentation, including evaluations, care plans, and progress notes to enhance continuity of care.
• Supervising and guiding Physical Therapy Assistants in the delivery of care as needed, ensuring compliance with state regulations.
• Actively participating in interdisciplinary team meetings to contribute to integrated treatment plans and client outcomes.
• Regularly recording and reporting changes in the client's condition and progress and recommending necessary adjustments to treatment plans.

Patient Population / Client Focus

The client population you will serve includes adults and older adults requiring rehabilitation services following surgeries, chronic illness management, and general mobility challenges. A focus on individual-centered care is crucial, as you will work to develop trust and rapport, enabling clients to achieve their rehabilitation goals in a comfortable and familiar setting.

Qualifications

To qualify for this role, you must possess:

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Current CPR certification.
• A clean driving record and reliable transportation with valid automobile insurance.

Preferred Experience

A minimum of two years of experience in physical therapy, preferably within home health care or related settings is highly desirable. Experience working collaboratively with multidisciplinary teams will be valued.
