Key Responsibilities

In this role, you will be responsible for:

• Collaborating with healthcare professionals to develop and implement comprehensive treatment plans based on psychosocial assessments.
• Documenting patient progress through maintaining accurate clinical records and progress notes in compliance with clinic policies.
• Providing crisis intervention and counseling to help patients navigate emotional and behavioral challenges.
• Communicating effectively with physicians regarding patient needs, concerns, and progress to ensure continuity of care.
• Coordinating treatment and discharge planning with patients' families or support systems to promote successful transitions.
• Participating in program planning and contributing to the organization of clinical services.

Patient Population / Client Focus

You will work primarily with a diverse population of high-risk patients, including those with recent behavioral health crises, chronic mental health issues, and those needing immediate intervention. The role requires sensitivity and adaptability to meet the unique cultural and emotional needs of each patient, ensuring they receive the highest standard of care.

Qualifications

To be considered for this position, candidates must possess:

• A minimum of 2 years of clinical experience in an outpatient setting, demonstrating competency in delivering mental health services.
• A Master's or Doctorate degree in Social Work, Psychology, or a related field from an accredited institution.
• An active state licensure as a Licensed Professional Counselor (LPC) in Pennsylvania.
• Strong communication skills, both written and verbal, to effectively interact with patients and the healthcare team.
• The ability to work independently while managing a varied caseload efficiently.
• Proficiency in using Microsoft Office Suite and familiarity with electronic health record systems.

Preferred Experience

Experience in a community health setting and familiarity with managing high-risk patients would be advantageous. Additionally, knowledge of therapeutic modalities and frameworks that align with evidence-based practices is preferred.

Compensation and Benefits

We offer a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience. Our benefits package includes:
