This full-time position will focus on delivering high-quality rehabilitation services in the comfort of our patients' homes. Work Environment

This role will be situated primarily in patients' homes, allowing you to offer personalized therapy services in a familiar environment that encourages comfort and progress. You will be working collaboratively with a multidisciplinary team of healthcare professionals to provide holistic care that addresses both the physical and emotional needs of our patients. Operating on a Monday to Friday schedule, this position emphasizes work-life balance while allowing professionals to hone their skills in a supportive and innovative atmosphere.

Key Responsibilities

• Develop and implement customized physical therapy plans tailored to prevent disability, relieve pain, and restore muscle function as prescribed by a physician.
• Lead physical therapy assistants in providing care to ensure adherence to the established plan of treatment.
• Conduct comprehensive assessments of clients' physical abilities, utilizing diagnostic tests to determine their functional level.
• Evaluate and modify the home environment to enhance clients' independence and achieve rehabilitation goals.
• Utilize therapeutic techniques including manual therapy, therapeutic exercises, and advanced modalities such as traction and massage, offering effective solutions to alleviate pain and improve recovery outcomes.
• Monitor and document patients' progress meticulously, adjusting treatment plans as necessary based on ongoing evaluations and patient feedback.
• Collaborate with interdisciplinary team members during case conferences to streamline care and ensure seamless communication between all parties involved in the treatment process.
• Educate clients and their families about the therapy process, including the effective use of physical therapy devices and techniques to continue recovery outside of therapy sessions.
• Adhere to all documentation requirements, including the timely submission of clinical records such as initial evaluations, progress notes, and discharge summaries.
• Assist in the supervision and training of physical therapy assistants, ensuring quality care and compliance with relevant state laws and regulations.
• Perform on-site functional assessments regularly to evaluate care plan efficacy and adjustments based on patient needs.
• Engage in continuous professional development and stay abreast of best practices in physical therapy.

Patient Population / Client Focus

Our patient population primarily includes individuals recovering from orthopedic surgery, injury rehabilitation, and chronic pain management. You will be instrumental in helping clients of varying backgrounds regain mobility and independence, managing challenges in diverse home settings. Your expertise will have a direct impact on their recovery journey, enhancing their quality of life.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current valid license to practice physical therapy in the state of Florida.
• Minimum of 2 years of relevant experience in a similar role, preferably in a home care setting.
• Basic Life Support (BLS) or CPR certification up to date.
• Valid driver's license and automobile insurance, enabling travel to patient locations.
• A strong commitment to clinical excellence, patient care, and ethical conduct.

Preferred Experience

• Experience working with an interdisciplinary healthcare team.
• Background in home health, hospice care, or similar environments.
• Familiarity with electronic medical record (EMR) systems to track patient data and progress efficiently.

Compensation and Benefits

The compensation for this position ranges from USD 100,000 to 120,000 annually, depending on experience and qualifications.
