Key Responsibilities\n

• Assist primary care providers in recognizing and managing mental health and psychosocial issues as a cohesive member of the primary care team.

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations and developing recommendations for treatment plans in collaboration with primary care providers.

• Provide brief, focused interventions for patients in need of mental health services, utilizing evidence-based practices and therapeutic techniques.

• Evaluate which patients are best served within the primary care setting and which should be referred to specialized mental health facilities.
\n
• Follow up with clients to monitor progress, making necessary adjustments to treatment approaches as indicated.
\n
• Develop individualized relapse prevention plans to promote stable functioning and decrease the likelihood of recurrence of mental health issues.
\n
• Detect 'at risk' patients and formulate strategies to prevent the worsening of their conditions.
\n
• Coordinate health services related to behavioral health and work collaboratively to ensure comprehensive patient care.
\n
• Document client progress and diagnostic information meticulously in treatment charts, ensuring compliance with regulations and standards.
\n
• Provide educational resources and self-management skills to enable clients to actively participate in their treatment and recovery processes.
\n
• Consult with clinic management and team members to enhance behavioral health services and evaluate program outcomes.
\n
• Assist patients in adhering to medical treatments initiated by primary care providers and offer coping strategies for potential medication side effects.

• Uphold the highest ethical standards as outlined in the Code of Ethical Conduct for Clinical Professionals.

• Undertake additional duties as assigned that contribute to the overall success of the clinic.

Qualifications\n

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
