Key Responsibilities\n

• Deliver remote counseling through comprehensive 8-week evidence-based programs tailored to improve mental health outcomes.

• Carefully monitor participant progress, conducting follow-up communications, documenting session notes, and advising on care coordination as necessary.

• Actively participate as a pivotal member of the care team, helping participants navigate available resources and obtaining required consent and authorization forms.

• Engage in regular meetings with supervisors, contributing to monthly team discussions and corporate Town Halls aimed at enhancing service delivery.

Patient Population / Client Focus\n

Qualifications\n

• Active professional licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
