Key Responsibilities\n

As a Licensed Clinical Professional Counselor, you will:

• Assist primary care providers by identifying and managing mental health and psychosocial issues, actively participating in the interdisciplinary primary care team.

• Conduct comprehensive patient intakes focusing on diagnostic evaluations, recommending tailored treatment goals and action plans to healthcare providers.

• Deliver brief, focused therapeutic interventions for patients requiring mental health support, ensuring approaches are evidence-based and effective.
\n
• Monitor patient progress through consistent follow-ups, adjusting treatment plans as necessary to promote optimal recovery and stability.
\n
• Develop and implement relapse prevention strategies, while aiding patients in maintaining their daily functions.
\n
• Cascade support for high-risk patients by identifying warning signs and creating preventive strategies in collaboration with other healthcare professionals.
\n
• Coordinate and document the delivery of health services related to behavioral healthcare, ensuring comprehensive care by engaging with external treatment providers when permitted by patients.

• Educate patients in self-management techniques and coping strategies for any medical treatments initiated by primary care providers.

• Maintain thorough clinical records and prepare related reports to facilitate clear communication with other team members regarding treatment plans.

Qualifications\n

To be considered for this role, candidates must possess:

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 annually.
