Work Environment

This role is onsite at our hospital-affiliated clinic, which offers a collaborative and interdisciplinary work environment. Our facility is equipped with both modern medical technologies and a welcoming atmosphere, allowing healthcare professionals to focus on delivering exceptional patient care. Your operating hours will typically be from 9:00 a.m. to 6:00 p.m., with some evening clinics extending to 8:00 p.m. Participation in weekend community outreach events and health fairs may also be required, reflecting our commitment to accessibility and public health initiatives.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families within the community, employing evidence-based techniques to ensure comprehensive evaluations.
• Diagnose and manage both acute and chronic health conditions, providing preventive care strategies and health promotion interventions tailored to individual needs.
• Develop and oversee community health education programs in collaboration with local organizations, helping to bridge gaps in health literacy and access.
• Provide culturally sensitive care, actively advocating for patients’ rights and ensuring they have equal access to healthcare services.
• Coordinate referrals and ensure comprehensive follow-up care, thus enhancing continuity in patient management and support.
• Maintain meticulous and compliant medical records as per regulatory standards, utilizing electronic health record systems for accuracy and efficiency.
• Engage in community outreach, facilitating health fairs and screenings to enhance public health awareness and accessibility.
• Stay informed on emerging community health trends, health policies, and best practices to elevate the quality of care provided in our clinic.

Patient Population / Client Focus

Qualifications

• Valid certification as a Nurse Practitioner.
• Active nursing license within the state of Florida.
• A Master's degree in nursing from an accredited institution.
• At least 3-5 years of clinical experience, ideally in community or public health settings, demonstrating expertise in managing diverse patient populations.
• Exceptional interpersonal and communication abilities, enabling effective interaction with patients, families, and community stakeholders.

Preferred Experience

While not mandatory, preference will be given to candidates with prior experience working in family health or cardiology settings, as well as those who have a proven track record in developing health education initiatives or participating in community-based programs. Compensation and Benefits
