Position Overview

Trails Rehabilitation & Wellness Pro is looking for a dedicated and skilled Licensed Physical Therapist (PT) to join our esteemed rehabilitation facility located in Woodbridge, Virginia. This full-time role requires a compassionate individual with a strong foundation in orthopedic physical therapy and at least two years of relevant experience. You will play a crucial role in enhancing patient mobility and recovery by implementing individualized treatment plans.

Work Environment

This position is based onsite at our modern rehabilitation facility in Woodbridge, Virginia. The facility is equipped with state-of-the-art tools and technology designed to support rehabilitation efforts and enhance patient outcomes. You will join a team of dedicated healthcare professionals in a supportive environment where open communication and teamwork are highly valued. Your work schedule will be Monday through Friday, from 8:00 am to 5:00 pm, allowing for a healthy work-life balance.

Key Responsibilities

• Assess patients' physical abilities by reviewing their history, conducting specific assessments, and developing comprehensive treatment plans tailored to their individual needs.
• Implement treatment protocols based on evaluation findings—this includes adapting techniques as necessary to cater to the patients' response and current health care practices.
• Maintain accurate, timely documentation of patient care activities which includes detailed initial evaluations, daily encounter notes, interim progress summaries, and discharge evaluations.
• Consult with other healthcare providers to ensure a holistic approach to patient care and promptly address changes in patient conditions.
• Participate in discharge planning and follow-up care to ensure continuity in rehabilitation and support for the patient post-therapy.
• Engage actively in facility meetings and interdisciplinary teams to help remove barriers to care and improve access to rehabilitation services.
• Adhere to all state and federal regulations regarding physical therapy practice, including maintaining compliance with continuing education requirements.

Patient Population / Client Focus

Your work will primarily focus on patients with orthopedic needs, including those recovering from surgeries, injuries, or chronic conditions impacting mobility. This diverse clientele ranges from young athletes to seniors, requiring an adaptable approach to treatment and rehabilitation strategies. Your role will substantially contribute to enhancing their quality of life through effective physical therapy interventions.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• Active state licensure as a Physical Therapist from the Virginia Physical Therapy Examining Committee.
• Two years of outpatient orthopedic experience, demonstrating proficiency in managing a wide range of musculoskeletal issues.
• Current certification in CPR to ensure readiness in emergency response situations.

Preferred Experience

While the minimum required experience is two years, candidates with experience in specialized orthopedic settings, advanced evaluation techniques, or multi-disciplinary teams will be regarded favorably. Experience in utilizing electronic health records and rehabilitation software is also a plus.

Compensation and Benefits
