Work Environment

This is a full-time, onsite position situated within a home care setting, allowing you to provide personalized treatment to patients in their environments. Key Responsibilities

• Conduct detailed evaluations of patients, which include reviewing medical histories, conducting specific assessments, and documenting treatment plans tailored to individual needs.
• Effectively communicate evaluation findings and treatment objectives to patients and their families, fostering their involvement in the care process and goal setting.
• Implement evidence-based treatment protocols based on evaluation results and each patient's unique presentation, adapting to their specific needs and progress.
• Consult and collaborate with an inter-professional healthcare team to ensure comprehensive patient care and optimal treatment outcomes.
• Recognize and document any changes in patient conditions, adjusting treatment plans as necessary and ensuring continuity of care.

Qualifications

Compensation and Benefits

We offer a competitive salary range of $100,000 – $120,000 annually, commensurate with experience and qualifications.
