This role will operate in a remote setting, providing ample flexibility while allowing our counselors to connect with clients in their home environments. We are rooted in Portland, Oregon, but serve clients across various regions. Our outpatient office approach requires both individual and collaborative contributions to treatment plans, ensuring that our counselors have the resources and support they need for success.

Key Responsibilities\n

• Conduct ongoing assessments to determine the unique needs of each patient, adjusting treatment plans based on feedback and observed progress.

• Communicate effectively with supervisors, direct care staff, family members, and guardians to share insights regarding individual client progress and therapy outcomes.

• Develop and provide a tentative schedule to supervisors, detailing appointments, home visits, and observations to maintain transparency around work progress.

• Organize and coordinate home visits, ensuring compliance with observation requirements, chart reviews, and necessary data analysis for effective client treatment.

• Ensure the implementation of required program plans is both effective and aligns with clinical best practices.
\n
• Participate in interdisciplinary team meetings to foster collaboration and share strategies that enhance client care.
\n
• Document and complete comprehensive monthly and quarterly progress notes to track treatment outcomes and client engagement.
\n
• Deliver training to staff members to ensure competence in the execution of program plans and documentation protocols.
\n
• Provide supportive follow-up through interviews, home visits, and observations to reinforce patient care.

• Maintain accurate records to guarantee all relevant information is accessible to interested parties while upholding client confidentiality.

• Serve as a liaison with the public and mental health community, advocating for the development of goals that support client needs.

• Assist in crafting training materials aimed at improving procedures and programs tailored for individual client enhancement.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted license as a Licensed Professional Counselor (LPC) in the state of Oregon is mandatory.
