Key Responsibilities\n

• Conduct ongoing assessments to evaluate the mental health needs of individual patients, developing tailored intervention strategies based on their unique circumstances.

• Maintain clear and consistent communication with supervisors, direct care staff, family members, and other involved parties to discuss patient progress and collaborative strategies.

• Provide supervisors with a well-organized schedule of appointments, home visits, and observations to track and support patient advancement and program adherence.

• Coordinate home visits to conduct in-depth observations, participate in chart reviews, and interview staff, gathering key data to inform treatment decisions.
\n
• Ensure effective implementation of treatment plans by actively participating in and contributing to Interdisciplinary Team Meetings.
\n
• Document monthly and quarterly progress notes to maintain accurate patient records and facilitate continuity of care.
\n
• Organize and lead training sessions to ensure staff competency in program implementation and documentation requirements.
\n
• Provide follow-up interventions, including home visits and observations, to address ongoing patient needs and provide continued support.
\n
• Act as a liaison within the mental health community, fostering relationships that support program development and community education initiatives.

• Assist in creating procedural training materials needed to improve programs for clients and enhance the staff's skills.

This role focuses on a diverse range of clients, including children, adolescents, and adults who may be experiencing challenges related to trauma, family dynamics, substance misuse, or behavioral issues. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
