Key Responsibilities\n

• Conduct ongoing assessments to identify and address the unique needs and challenges of each patient, ensuring tailored therapeutic approaches are implemented.

• Facilitate effective communication with supervisors, direct care staff, family members, and other stakeholders about patient progress and treatment plans.

• Prepare and submit a comprehensive schedule of appointments and home visits to supervisors for effective monitoring of client progress.

• Coordinate home visits when necessary to review charts, conduct staff interviews, and analyze data pertinent to patient care.
\n
• Ensure the meticulous implementation of evidence-based program plans, providing necessary adjustments to improve therapeutic outcomes.

• Actively participate in Interdisciplinary Team Meetings and relevant discussions to collaborate on patient care strategies.
\n
• Complete monthly and quarterly progress notes, documenting crucial developments in each patient’s treatment journey.
\n
• Organize training sessions for colleagues to enhance their competency in implementing program plans and maintaining documentation standards.
\n
• Conduct follow-up interviews and home visits to reinforce support for patients, aiming to facilitate continued progress.
\n
• Maintain organized records that ensure timely access to information for involved parties while fulfilling legal and ethical standards.

• Serve as a liaison between patients, their families, and the community mental health professionals to advance developmental goals.

• Assist in creating and enhancing training materials to elevate program effectiveness and client engagement.

Patient Population / Client Focus\n

Our diverse client base includes both adults and youth, with particular emphasis on those facing challenges related to family dynamics, trauma, and substance use disorders. Qualifications\n

• Must possess a Master's or Doctoral degree in Counseling, Psychology, or a related field.
