Key Responsibilities\n

• Perform comprehensive assessments to tailor treatment plans to meet each patient's unique needs, ensuring continuous care and support.

• Facilitate regular communication among supervisors, family members, and caregivers regarding patient progress and therapeutic goals, fostering a team-oriented approach.

• Develop a structured schedule for appointments, home visits, and necessary observations to effectively monitor work advancement.
\n
• Coordinate and conduct home visits as needed, executing in-home observations and chart reviews to assess treatment implementation.

• Ensure adherence to program plans, employing best practices in therapeutic delivery and documentation standards.
\n
• Engage actively in Interdisciplinary Team Meetings, contributing insights on patient progress and treatment efficacy.
\n
• Document monthly and quarterly progress notes to track developments and inform necessary adjustments in care strategies.
\n
• Organize and lead training sessions for staff, ensuring that all team members are well-versed in program plans and documentation requirements.

• Provide ongoing support through follow-up interviews and home visits, adapting approaches based on client feedback and evolving needs.

• Act as a liaison with community mental health resources, collaborating to support patients in achieving their developmental goals.

Patient Population / Client Focus\n

Your role will encompass working with a diverse range of clients, including both adults and youth. You will have the opportunity to engage with individuals facing various mental health challenges, including trauma, substance misuse, and familial issues, providing empathetic and culturally competent care.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Oregon.

• A minimum of 2 years of counseling experience in a clinical environment is essential.

• A robust understanding of legal and ethical standards in counseling practices.

• Excellent interpersonal and communication skills to build rapport effectively with clients and their families.

• Demonstrated ability to work collaboratively within a multidisciplinary healthcare team.

• Proficient in using Electronic Medical Records (EMR) software and other relevant technologies.

Preferred Experience\n

• Specialization in family therapy, couples therapy, substance misuse counseling, or trauma-informed care is highly preferred.
