Work Environment\n

This position offers a flexible work environment primarily conducted remotely, allowing for greater work-life balance while addressing the needs of clients directly where they are most comfortable. As a part of the hospice care team, you will collaborate with healthcare professionals, caregivers, and family members to ensure that the seniors receive a coordinated and compassionate approach to their care. While the primary hours are Monday to Friday from 8:00 AM to 4:30 PM, there may be occasions requiring evening or weekend hours based on client needs, requiring flexibility and a commitment to on-call support.

Key Responsibilities\n

• Conduct thorough psychosocial assessments of seniors, capturing essential emotional, mental, and social factors that affect their overall well-being.

• Collaborate closely with clients and their families to develop individualized care plans tailored to their specific goals, preferences, and life situations.

• Provide ongoing counseling and emotional support, guiding seniors through life transitions and addressing complex family dynamics.

• Advocate for clients’ rights, ensuring they have access to necessary resources, services, and benefits within the community.

• Perform regular home visits to evaluate living conditions, monitor progress, and modify care plans as needed.

• Maintain accurate documentation, including assessments, care plans, and progress notes, in compliance with ethical and legal standards.

Qualifications\n

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 60,000 to 100,000 annually, based on experience and qualifications.
