Work Environment

The work setting for this role will be primarily onsite within the home environments of our clients in Henrico, Virginia. The position requires flexibility and adaptability, as you will be assessing and treating patients in various locations, including their homes. Key Responsibilities

• Conduct thorough evaluations, including assessments of patient history, physical condition, and personal goals to determine the most effective treatment protocol.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in goal-setting for optimal engagement and success.
• Implement tailored treatment strategies based on patient evaluations, ensuring methods align with current best practices and guidelines in physical therapy.
• Collaborate with other healthcare professionals to coordinate comprehensive care, making timely adjustments to treatment plans as needed based on patient progress.
• Participate in discharge planning and provide ongoing follow-up care, meticulously documenting each step to create a clear record of patient progress, including daily notes and interim evaluations.
• Manage time effectively to meet productivity standards, attend meetings punctually, and participate in community outreach through marketing activities that promote wellness.
• Contribute towards department objectives and goals, adhering to governmental regulations and accreditation requirements.
• Proactively engage in continuing education to enhance professional development and maintain competency standards for both self and the organization.
• Ensure high-quality customer service, maintaining access flexibility to meet patient needs efficiently.
• Participate in initiatives aimed at improving access to care and understanding site-specific performance measurements.

Patient Population / Client Focus

This position primarily serves a diverse group of patients needing physical rehabilitation in the Henrico community. Clients may include elderly individuals recovering from surgeries, patients managing chronic pain, or those in need of rehabilitation following acute injuries. The ability to create personalized plans that resonate with each individual's circumstances is essential for promoting recovery and enhancing quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current and valid license to practice physical therapy in Virginia, as needed.
• Certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.

Compensation and Benefits
