Key Responsibilities

As a Home Care Physical Therapist, your responsibilities will include:

• Directing personalized physical therapy treatments aimed at alleviating pain, preventing disability, and enhancing muscle function in alignment with established care plans.
• Overseeing and mentoring Physical Therapy Assistants, ensuring the effective implementation of therapy programs tailored to individual client needs.
• Conducting thorough evaluations of the client's functional abilities using diagnostic tests to craft appropriate treatment strategies.
• Assessing home environments for rehabilitation challenges and providing actionable recommendations to enhance the client's independence.
• Delivering prescribed physical therapy services, including manual therapeutic exercises, traction, massage, and application of physical agents.
• Adapting and fitting prosthetic and orthotic devices as necessary and communicating modifications to the supervising physician.
• Contributing actively to interdisciplinary team discussions, sharing insights and information that support comprehensive patient care.
• Maintaining meticulous clinical records, including evaluations, care plans, and progress notes to ensure compliance with regulatory standards.
• Instructing clients and their families on the proper use and care of therapy devices and techniques to maximize treatment outcomes.
• Supporting case management through timely documentation and adherence to care protocols.

Patient Population / Client Focus

Your role will encompass working with a diverse patient population, including those recovering from surgery, managing chronic conditions, or undergoing prolonged rehabilitation following injuries. You will be integral to developing trusting relationships that foster adherence to treatment plans and emotional support through every step of their recovery.

Qualifications

The ideal candidate will possess the following qualifications:

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and valid PT licensure in the state of Virginia, in good standing.
• Active CPR certification.
• Exceptional interpersonal skills, with a strong ability to engage and communicate effectively with clients and their families.
• A proactive attitude and the ability to work both independently and as part of a collaborative healthcare team.
• Creative problem-solving capabilities and a commitment to clinical excellence and quality care.

Preferred Experience

While we welcome applications from all qualified candidates, the following experience is highly desirable:

• A Doctorate in Physical Therapy (DPT).
• A minimum of one year of experience in physical therapy within acute care or home care settings.
• Previous experience in supervising or mentoring junior staff, particularly Physical Therapy Assistants.
• Familiarity with federal, state, and governing agency regulations related to home health services.

Compensation and Benefits

This full-time position offers a competitive salary range of $90,000 to $100,000 annually, commensurate with experience.
