Work Environment

Key Responsibilities

• Conduct comprehensive evaluations, including reviewing medical histories, performing assessments, and formulating treatment recommendations based on findings.
• Communicate evaluation results and treatment plans clearly to patients and their families, collaborating with them to set achievable goals.
• Implement treatment protocols adapted to each patient's unique needs, ensuring alignment with the evolving standards of care in physical therapy.
• Regularly consult with interdisciplinary healthcare professionals to ensure comprehensive patient care and necessary modifications to treatment plans.
• Manage discharge planning effectively and ensure thorough follow-up care, documenting all interactions and patient progress accurately and timely.
• Exhibit strong time management skills, maintaining an organized schedule that meets productivity goals and departmental objectives.
• Participate in community outreach and marketing efforts to promote the availability of our services and remove barriers to patient access.

Patient Population / Client Focus

Your patient caseload will primarily consist of individuals recovering from surgeries, injuries, or chronic conditions who are in need of rehabilitative support within their home environments. This role requires adaptability and a patient-centered approach, as each patient will present with unique challenges and similar goals of gaining independence and improving quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution is required.
• A current and valid license to practice Physical Therapy in Virginia.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A commitment to professional growth through ongoing education and skills enhancement.

Preferred Experience

• While prior work experience is not required, any background in outpatient or home care settings will be advantageous.

Compensation and Benefits
