Key Responsibilities\n

As a Licensed Clinical Professional Counselor, your responsibilities will include, but are not limited to:

• Serving as an essential member of the primary care team, assisting providers in identifying and managing patients' mental health and psychosocial issues.

• Conducting comprehensive patient intakes, utilizing diagnostic and functional evaluations to inform treatment plans and recommendations.

• Providing brief interventions and crisis management for patients seeking mental health support.
\n
• Making informed recommendations on treatment goals and referring patients to specialized services when appropriate.

• Monitoring patient progress and ensuring the timely follow-up of treatment outcomes and adjustments to care when necessary.

• Developing individualized relapse prevention plans to support ongoing mental health stability.
\n
• Documenting patient progress and treatment plans meticulously in compliance with agency policies and Federally Qualified Health Center (FQHC) guidelines.

• Educating patients in self-management techniques and coping strategies for better engagement in their recovery process.

• Consulting with clinic management and interdisciplinary teams on behavioral health services and program evaluation initiatives.

• Adhering to the Code of Ethical Conduct for Clinical Professionals while maintaining confidentiality and professionalism at all times.

Patient Population / Client Focus\n

The clientele at Crystal Lake Families & Kids Services spans various demographics, including children, adolescents, and adults facing a myriad of mental health challenges. As a counselor, you will work with patients who may be dealing with anxiety, depression, trauma, and other behavioral health concerns, providing a supportive and therapeutic environment tailored to their unique needs.

Qualifications\n

The ideal candidate for this position holds a minimum of a Master's Degree in Clinical Counseling and is currently licensed as a Clinical Professional Counselor (LCPC). Essential qualifications include:

Compensation and Benefits\n

The annual salary range for this role is between USD 87,000 and 105,000, commensurate with experience and qualifications.
