Work Environment\n

This position offers the flexibility of a remote work setting, allowing our LCSW to provide high-quality care from the comfort of their home or any preferred location in Jacksonville, Florida. You will collaborate closely with a multidisciplinary team focused on delivering comprehensive support to our pediatric patients and their families.

Key Responsibilities\n

• Conduct thorough evaluations and assessments of children’s developmental needs, employing both standardized tools and observational techniques.

• Engage in collaborative development and implementation of Individualized Family Service Plans (IFSP) tailored to the unique needs of each child.
\n
• Monitor and document progress on outcomes and ensure continual communication with team members and families to adjust strategies as necessary.
\n
• Provide critical emotional and educational support to families, empowering them with the skills and knowledge to foster their child’s development.
\n
• Adhere to all applicable Federal and State regulations, maintaining compliance in documentation and service delivery.
\n
• Utilize evidence-based practices and the Natural Learning Environment Practices approach to create effective learning opportunities for children.
\n
• Foster rapport with children and their families to establish a trusting and supportive client-provider relationship.
\n
• Provide guidance to families regarding strategies to enhance their child’s developmental skills across various domains.
\n
• Utilize coaching models and natural environment teaching methods to facilitate learning and coping strategies.
\n
• Ensure the health and safety of all individuals involved by upholding the highest standards of practice.

Patient Population / Client Focus\n

As an LCSW in our hospice center, you will focus primarily on serving infants and toddlers who may have disabilities or developmental delays during their critical early years. Your work will involve direct interactions with children and their families, providing support that aims not just to meet developmental challenges, but to enrich their quality of life during vulnerable times.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) recognized by the Department of Public Health.

• A valid driver's license is required for occasional travel to clients’ locations.
\n
• A minimum of one year of direct experience working with children in early intervention services.
\n
• Demonstrated commitment to Natural Learning Environment Practices and evidence-based methodologies.

Compensation and Benefits\n
