Key Responsibilities\n

• Conduct thorough psychosocial assessments and evaluations for clients to identify their needs and develop appropriate care plans.

• Engage with clients and their families in collaborative development of Individualized Service Plans (ISPs), ensuring that interventions meet the unique needs of each individual.

• Monitor and track treatment outcomes, actively participating as a member of an interdisciplinary team focused on holistic, patient-centered care.

• Provide education and support to families, helping them understand the challenges of aging and the resources available to them.

• Ensure compliance with all relevant Federal and State regulations in the delivery of clinical services.

• Utilize evidence-based therapies and interventions to promote optimal mental health outcomes.

• Build rapport with clients and their families, fostering a trusting therapeutic alliance that enhances the effectiveness of interventions.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is required.
